In this section, you will focus on the different processes or behaviors that can potentially result in unintentional harm, medical errors, or other consequences to a patient because of human factors and behaviors by the healthcare staff or other personnel. You will then identify a quality improvement tool of your choice and alert the staff of the tool, as well as what it monitors. You may choose to include graphics or charts to explain the tool.
Section 3 of the handbook
You will begin by providing a brief overview of the concept of total quality management and continuous quality improvement. You will then provide a visual and written description of each of the three common models used: FOCUS-PDCA, Rapid Cycle Improvement, and Six Sigma. Beneath each explanation, you will discuss under what circumstances the staff may be expected to apply a particular tool and how to do so effectively, thus resulting in reduced chance of risk to the patients, staff, and organization.
Section 4 of the handbook
You will begin by providing an overview of why we must identify risks in the health care setting and why it is important for each member of the organization to be involved in the identification and in taking action for addressing the risks, all of which should ultimately result in a reduction of overall risk. You should then introduce and describe at least two different tools that can be implemented to improve performance and reduce risk. Visual tools may be included.
Section 5 of the handbook
You will identify some of the different process opportunities that may exist within a healthcare organization and why it is important for progressive health care organizations to address these challenges. You will then discuss what considerations need to be made to address the challenge including, but not limited to, the size of the team, specific positions and roles of each member, creation of a problem statement and team charter, and creation of a draft agenda and Gantt Chart of timeline. Visual graphics should be included to support your explanation of each of these considerations so that they can be replicated by staff.
Section 6 of the handbook
Your goal for this section is to enlighten the staff of some of the different challenges that the risk manager faces and why it is so important for all staff to become familiar with how they can apply risk analysis techniques and tools to ensure that risk reduction strategies are being applied in all aspects of patient care. Explain and justify four to six different strategies that could be implemented to prevent such events from occurring.
