AGGREGATE CLINICAL PRACTICUM PAPERGRADING CRITERIA
The aggregate clinical practicum will be evaluated by a report from the clinical preceptor and a written
summary of the experience to be due at the end of the final week of this experience. This report should
reflect an assessment of the experience and analysis of the application of community health concepts in
the program, as described below:
- DESCRIBE THE STRUCTURE OF THE HEALTH CARE DELIVERY SYSTEM (15pts)
Physical characteristics of the health care delivery system (ie: clinic)
Organizational structure, staffing, roles performed of the health care team
List the major healthcare related programs or services offered.
- DESCRIBE THE CLIENT SYSTEM (10pts)
Demographics: use statistics to describe
Identify three relevant nursing diagnoses for the client aggregate, based on your analysis of the client
System. Must reflect assessment data.
- DESCRIBE THE ROLE/FUNCTIONS OF THE NURSE (15pts)
What is the structure of a usual day? Hours, scheduled activities, # clients seen, etc.
What nursing activities are routinely performed in the provision of care?
What management roles/organizational activities are required in coordination of care within the agency?
- Define the following terms and describe specific examplesof activities you observed or discussed in
whichthe nurse, other providers, or the program implements each of the following roles: (If a role was not
observed or discussed, suggest how it could be implemented)
- Individual Client-oriented roles in providing care (15pts)

- Describe how knowledge of risk factors is used in planning and providing care to this aggregate. Give 2 specific examples. (10pts)
- Identify activities/programs according to the levels of care (3 levels of intervention) that are practiced: (5pts)
- Disaster Nursing (25 pts)
What is the disaster plan for this organization? Identify how many plans are in place? (5 pts)
Where is the disaster plan kept? (2.5 pts)
Describe the process of development. Who is involved? (7.5 pts)
Describe role of nursing in implementing this plan (10 pts)
- Evaluate in your own words the adequacy of this program in meeting the priority needs of theclient
system (5 pts)
Up to 10 points may be deducted from the final grade for deficiencies in typing, grammar, spelling, readability, etc. Please turn in this criteria sheet with
