COMMUNITY SETTINGS FOR FAMILY HEALTH NURSES

Overview

The family nurse provides care within many facilities in a community.  This is discussed in chapter 17 of your text.  They include nurse home visiting programs, community nursing centers, public health departments, and home education, and community education.  Discuss these facilities in your community as well as access to these resources.  For each resource be sure to include how families in the community benefit. Also discuss three ways in which a family health nurse may be useful in community resources.  Use what you’ve learned about the role of a family health nurse and what he/she may use to help in community settings.

Objectives

• Examine family health nurse role in various community health settings.

In the written assignment this week you will be discussing the facilities in your community and their resources. You will use the information from pages 570-576 in the 5^th edition (544-548/Chapter 17 in the 6^th edition) to discuss the family nurse role in providing care within community facilities.

• Cover programs such as: home visiting programs, community nursing centers, public health departments, and home education, and community education.  Every community has these types of resources.
• Discuss the facilities within your own community as well as access to these resources.
• For each resource, include how families in the community benefit from the services.
• Discuss 3 ways in which the family health nurse may be useful in community resources.
• Must include 4 references, 2 must be from course materials and 2 from peer reviewed references. As we are aware in week 5 there is only 1 required/recommended reference, please use the Kaakinen text and 3 additional peer reviewed references to meet the guidelines of the assignment
• Reference must be no older than 5 years (2015-2020) unless course material.

Some facilities Geauga County, Ohio offers.

1.University Hospital Geauga Medical Center – Benefits: Emergency care and surgical services close to home.

2.Skilled Nursing Homes (some with locked memory care units such as dementia, Alzheimer’s, etc.) Geauga Co. has many skilled facilities. Loved one is close to family allowing more frequent visits from family members.

4. Senior Day Center—through the county we have a senior day center where senior citizens can visit M-F 8a – 5p. They are given lunch and snacks. They can do many activities: arts & crafts, play cards, receive haircuts, toenails clipped, etc.  Provides socialization with other in their community.  Allows the adult children respite care and provides peace of mind knowing their loved one is safe while they are at work.

5.YMCA – Offers a rehabilitation program for post-operative joint replacement surgery. Provides a place to stay fit.

6.Geuaga Public Health—offers free childhood and adult immunizations.  They only serve the under insured or no insurance county residents.

4. Various mental health programs, and community health education programs.

7.Geauga County Group Home – offers residents to solely independent, low-income senior residents.

8. UH Geauga Medical Center has a Visiting Nurse Program to assist patients after surgery and a pain management program.
