This quality improvement evidence-based project will be to determine if implementing a HF education in a house call setting will improve self-care behavior and management of HF and, in turn, reduce hospital readmissions.
The Intervention will be to implement a HF prevention protocol using the Zoom and HF Path Heart Failure self-management education Tool.
Clinical Question Will the implementation of a heart failure prevention protocol encompassing staff training and education reduces the number of hospital readmissions in a house call practice?
The question that will be answered by this project:
P: The population is the nursing staff.
I: The Intervention will be to Implement an evidenced-based standardized heart failure protocol that uses staff education, discharge planning, medication management, and telephone follow-up.
C: A comparison of the number of patient readmission before and after implementation of the protocol.
O: Reduction of heart failure readmission in a house call practice.
Population/Setting The setting for this project will take place in a house call in a southeast rural area in Dallas.
A convenience sample will be utilized for selecting participants for this evidence-based project. The potential sample group will be nurses in the house call.
